Understanding PCOS

At its core, Polycystic Ovary Syndrome (PCOS) is a complex hormonal and metabolic imbalance. While the name suggests the presence of cysts on the ovaries, it is actually a family of symptoms rather than a single disease. As of May 2026, Polycystic Ovary Syndrome (PCOS) has been officially renamed to Polyendocrine Metabolic Ovarian Syndrome (PMOS). This update better reflects the true nature of the condition as a systemic metabolic and hormonal disorder, moving away from the misconception that it is merely a fertility issue centered on ovarian cysts.

Although PCOS doesn’t look the same for everyone, t
he prevailing Rotterdam Criteria shows three major symptoms of PCOS:
  • Irregular or Absent Ovulation: Usually appears as irregular periods or fewer than 8 cycles per year, often with period pain and abdominal discomforts.
  • Hyperandrogenism: High androgen levels. It can be detected either via blood tests or through physical signs (clinical) like excess facial/body hair, severe acne or hair loss.
  • Polycystic Ovarian Morphology (PCOM): Characterized by having 20 or more follicles in one or both ovaries, or an ovarian volume greater than 10ml.

And under these symptoms, there are two key drivers: insulin resistance and hyperandrogenism. When the body’s cells become resistant to insulin, the pancreas overcompensates by producing more of the hormone. The excess insulin directly signals the ovaries to ramp up androgen production, such as testosterone. High insulin levels also inhibits the liver from producing Sex Hormone-Binding Globulin (SHBG), a protein that usually “mops up” excess testosterone in the blood. This elevated level of androgens interferes with the signals between the brain and the ovaries. The disruption prevents follicles from maturing and releasing an egg, fundamentally arresting the natural reproductive cycle. Therefore, hyperandrogenism is not just a primary feature of PCOS, but also a causative factor for this condition.

  1. Thank you for this post. PCOS does not get much attention in the “medical” community.

    I was diagnosed with PCOS In the early ’80s. At this stage of my life my ovaries no longer function. The cysts and infertility are no longer a problem but all the problems that are a part of a PCOS diagnosis do not go away. PCOS is not just a “Reproductive” problem. It is a life long issue but fertility is the only part of the syndrome that doctors are concerned with. It’s not like everything stops when your ovaries shut down.That is why, when JW started taking peptide requests, I requested a peptide for PCOS. I see that, now, there is a second peptide for PCOD (Polycystic Ovary Disease). I’m not sure how the two problems are different but I noticed that the frequencies are different.

    Women with a PCOS diagnosis are at higher risk for breast and ovarian cancer. So my question is, can these peptides and/or Rife frequencies be safely used if the ovaries no longer function.

    Again, I thank you for this post and the Program suggestions. You have provided more information to me than any physician ever has. It has always seemed to me that doctors are only concerned with fertility and all the other issues are of no consequence.

    1. Hi Sharon! Thank you so much for sharing your experience. We agree with the fact that PCOS is a manifold syndrome rather than a mere fertility disorder and we hope to raise this awareness. If the ovaries are no longer functioning, the treatment should be focused on preventative metabolic care, such as persistent insulin resistance, androgen level control, and ensuing endometrial, breast & cardiovascular health issues.
